Overview
Alar reduction removes skin and tissue from the outer alar base (external excision) or inner nostril floor (internal excision) to narrow the nostril width. It is a relatively simple procedure, but scar placement and management are critical for natural-looking results.
Mechanism
The surgeon marks the planned excision area based on the desired degree of narrowing. In external (Weir) excision, a crescent of skin is removed from the alar-facial groove junction. In internal excision, tissue is removed from the nostril sill or floor. The incisions are closed with fine sutures, and careful scar placement along natural skin creases minimizes visibility. The amount of tissue removed is conservative to avoid an overly pinched or unnatural appearance.
