A surgery that increases breast size using implants (silicone gel, saline) or autologous fat transfer.

Overview

Breast augmentation with implants is the most common approach, with variations in incision location (axillary, periareolar, inframammary fold), implant placement (subglandular, submuscular, dual plane), and implant shape (round, anatomical/teardrop). In Korea, the axillary approach with dual-plane placement is popular. Autologous fat transfer is an alternative for modest augmentation with a more natural feel.

Mechanism

Implant augmentation: an incision is made at the chosen site, a pocket is created above or below the pectoralis major muscle, and the implant is placed. Dual-plane technique partially releases the lower muscle origin for natural implant settling. Fat transfer: fat is harvested via liposuction, purified, and injected into the breast tissue in multiple layers. Each method creates volume through different means but aims for natural shape and feel.

Effects

Increased breast size and fullness, improved breast shape and projection, enhanced cleavage, correction of breast asymmetry, and improved body proportion and confidence.

Duration

Implants: 10-20+ years (may require exchange); fat transfer: permanent but 30-50% volume absorption expected

Recovery

Significant soreness and tightness for 1-2 weeks. Compression bandage or surgical bra for 4-6 weeks. Avoid upper body exercise for 4-6 weeks. Swelling resolves over 1-3 months. Implants drop and settle into final position over 3-6 months.

Side Effects

Temporary soreness, swelling, and tightness. Risk of capsular contracture, implant rupture, implant displacement, asymmetry, rippling, loss of nipple sensation, infection, and BIA-ALCL (very rare). Fat transfer risks: fat necrosis, calcification, and volume loss.
