A surgery that reduces overly large breasts by excising excess skin, glandular tissue, and fat to decrease size and improve shape.

Overview

Breast reduction addresses both functional problems (shoulder pain, back pain, skin irritation) and cosmetic concerns. Techniques include vertical incision and inverted-T incision methods, both including nipple-areolar complex repositioning. Scar extent depends on the amount of tissue removed. The surgery significantly improves quality of life for patients with macromastia.

Mechanism

The surgeon marks a new, higher nipple position and designs the tissue excision pattern. Excess skin, glandular tissue, and fat are excised according to the plan. The nipple-areolar complex is transposed upward on a pedicle (maintaining blood supply and sensation). The remaining breast tissue is reshaped and the skin envelope is closed. The vertical method leaves a lollipop-shaped scar; the inverted-T method adds a horizontal scar in the inframammary fold.

Recovery

Surgical drains for 1-3 days. Surgical bra for 4-6 weeks. Moderate soreness for 1-2 weeks. Avoid heavy lifting for 4-6 weeks. Swelling resolves over 2-3 months. Scars mature over 6-12 months.

Side Effects

Scarring (inevitable), temporary swelling and bruising, changes in nipple sensation (temporary or permanent), risk of asymmetry, fat necrosis, wound healing problems, infection, and inability to breastfeed (depending on technique).
