A semi-permanent filler containing polycaprolactone (PCL) microspheres in CMC gel, providing immediate volume while stimulating collagen production.

Overview

Ellansé is a CE-certified filler developed by AQTIS Medical (Netherlands), consisting of PCL (polycaprolactone) microspheres uniformly distributed within a CMC (carboxymethylcellulose) gel carrier. It simultaneously provides immediate volume (CMC gel) and long-term collagen stimulation (PCL). Available in S (1 year), M (2 years), L (3 years), and E (4 years) types based on duration, with S and M types most commonly used in Korea.

Mechanism

The CMC gel provides immediate physical volume while PCL microspheres stimulate collagen neogenesis in surrounding tissue. The CMC gel is absorbed within weeks, but collagen induced by PCL replaces the volume. PCL is gradually broken down (hydrolysis) and excreted as water and CO2.

Effects

Immediate volume is visible upon injection, transitioning to natural collagen-based volume over 1–3 months. S-type lasts approximately 1 year; M-type approximately 2 years.

Duration

15–30 minutes per area.

Recovery

Normal activities can resume immediately. Swelling and bruising may appear for 1–3 days.

Side Effects

Swelling, bruising, and pain are common. Unlike HA fillers, Ellansé cannot be dissolved with hyaluronidase, making overcorrection difficult to reverse. Choosing a skilled practitioner is crucial. Nodules and asymmetry may rarely occur.
