A surgery that removes the epicanthal fold (Mongolian fold) to expose the inner corner of the eye and widen the horizontal eye width.

Overview

Epicanthoplasty (medial canthoplasty) incises and repositions the epicanthal fold covering the inner corner of the eye to expose the medial canthus. It improves the appearance of widely spaced or stuffy-looking eyes. Techniques include Z-plasty, W-plasty, and skin redraping methods. Often combined with double eyelid surgery for comprehensive eye reshaping.

Mechanism

The surgeon incises the epicanthal fold using geometric patterns (Z-plasty or W-plasty) that redistribute skin tension and prevent scar contracture. The skin covering the medial canthus is carefully repositioned to expose the lacrimal caruncle (pink tissue at the inner eye corner) without creating visible scarring. The key is to release the vertical tension of the epicanthal fold while creating a natural, scar-free inner eye corner.

Effects

Wider horizontal eye width, exposed inner eye corner, reduced inter-eye distance appearance, brighter and more open eye look, and improved eye proportion and balance.

Permanent; minimal regression expected

Recovery

Swelling and bruising for 1-2 weeks. Stitches removed at 5-7 days. Inner corner area remains pink and slightly raised for 1-3 months. Scar maturation takes 3-6 months. Scar concealer makeup possible after stitch removal.

Side Effects

Temporary swelling, bruising, and inner corner redness. Risk of visible scarring (most common concern), web formation (scar contracture), over-correction (exposing too much pink tissue), and asymmetry. Scar management (silicone gel) important.
