A surgery that harvests fat from other body areas (abdomen, thighs, etc.) via liposuction, purifies it, and injects it into areas needing volume.

Overview

Fat grafting uses autologous tissue, eliminating foreign body reactions as its greatest advantage. It is applied to the entire face (forehead, cheeks, temples, chin, lips) as well as breasts and buttocks. The fat survival rate (30-70%) is the key variable, and once engrafted, the fat remains permanently. Over-grafting or uneven engraftment may require correction procedures.

Mechanism

Fat is harvested from donor sites via liposuction using small cannulas with gentle negative pressure. The harvested fat is purified through centrifugation, filtration, or decanting to remove oil, blood, and damaged cells. The refined fat is then injected in small aliquots across multiple tissue planes (micro-fat grafting technique) to maximize contact with blood supply and improve survival rates. Nanofat (further processed, finer fat) may be used for superficial skin quality improvement.

Recovery

Side Effects

Temporary swelling and bruising at both sites. Risk of fat necrosis (hard lumps), calcification, over-grafting, asymmetry, oil cyst formation, uneven absorption, and donor site contour irregularity. Fat embolism is extremely rare but potentially serious.
