Overview

Filler is a non-surgical procedure that injects gel-type filling materials beneath the skin to add volume to hollow areas, smooth wrinkles, and refine facial contours. The most widely used component is hyaluronic acid (HA), with other types including calcium hydroxylapatite (CaHA), polycaprolactone (PCL), and poly-L-lactic acid (PLLA). Common treatment areas include nasolabial folds, cheeks, chin, forehead, nose, lips, and under-eye hollows.

Mechanism

When the gel-type material is injected into the subcutaneous or dermal layer, it physically fills the space to create an immediate volume effect. HA fillers attract hundreds of times their weight in water, providing hydration to surrounding tissues. Some fillers (Sculptra, Ellansé) stimulate collagen production for long-term volume effects.

Effects

HA fillers show immediate volume improvement upon injection, lasting 6–18 months. Duration varies by product and treatment area—lips tend to be shorter (6–9 months) while cheeks and chin last longer (12–18 months).

Recovery

Normal activities can resume immediately. Mild swelling or bruising at the injection site may last 1–3 days. Avoid alcohol, saunas, and vigorous exercise on the day of treatment.

Side Effects

Temporary swelling, bruising, and redness are common and typically resolve within 1–3 days. Rare but serious complications include vascular occlusion, skin necrosis, and vision impairment—treatment by a specialist with thorough anatomical knowledge is essential. HA fillers can be dissolved with hyaluronidase, but non-HA fillers are difficult to reverse.
