A surgery that corrects the chin (mentum) in terms of length, projection, retrusion, and asymmetry.

Overview

Genioplasty horizontally osteotomizes the chin bone and moves the segment in the desired direction: advancement, setback, shortening, lengthening, or lateral shift. While simple augmentation can be done with a silicone implant, osteotomy provides more precise correction. All approaches are intraoral, leaving no external scars.

Mechanism

The surgeon accesses the chin through an intraoral incision, exposes the mental bone below the tooth roots, and performs a horizontal osteotomy. The freed bone segment is repositioned as planned (advanced for weak chins, set back for protruding chins, shortened or lengthened as needed) and fixed with titanium plates and screws. For narrowing, a wedge of bone may be removed centrally (T-osteotomy) before bringing the segments together.

Effects

Corrected chin projection, improved facial balance and proportion, correction of chin asymmetry, improved profile harmony, and more balanced lower facial contour.

Duration

Permanent; bone healing complete at 2-3 months

Recovery

Moderate swelling for 2-3 weeks. Chin numbness for weeks to months (mental nerve stretching). Soft diet for 1-2 weeks. Compression garment may be used. Return to work at 1-2 weeks. Final result at 3-6 months.

Side Effects

Temporary swelling, bruising, and chin numbness (very common, usually temporary). Risk of prolonged or permanent numbness, asymmetry, bone non-union, infection, tooth root damage, implant-related issues (if used), and chin drooping (mentalis muscle detachment).
