Overview

The incision method cuts the eyelid skin along a designed line to remove excess skin, orbicularis muscle, and orbital fat, then directly sutures the skin to the tarsal plate (levator aponeurosis) to form a permanent adhesion. It is suitable for patients with thick skin, excess fat, or drooping. Results are more permanent and defined than the non-incision method, but recovery is longer.

Mechanism

The surgeon incises along the pre-marked crease line, excises excess skin, orbicularis muscle, and protruding orbital fat as needed, then directly sutures the skin edge to the tarsal plate or levator aponeurosis. This creates a strong, direct adhesion between the skin and underlying structure. The resulting scar within the fold line becomes the permanent double eyelid crease.

Recovery

Side Effects

Temporary significant swelling and bruising. Visible scar (hidden in fold). Risk of asymmetry, over-correction (fold too high), under-correction, scar contracture, dry eyes, and prolonged swelling. More invasive than non-incision with longer recovery.
