Overview
Inverted nipples are caused by short or fibrotic milk ducts beneath the nipple that pull it inward. Severity ranges from mild (protrudes with stimulation) to severe (cannot protrude even with stimulation). Surgery involves a small incision at the nipple base to release the shortened ducts and fibrous tissue, then securing the nipple in an outward position with internal sutures.
Mechanism
The shortened lactiferous ducts and fibrous tissue bands that pull the nipple inward are identified and released. Internal sutures (purse-string or mattress sutures) secure the nipple in the everted position. Tissue flaps may be created to fill the space beneath the nipple and prevent re-inversion. Techniques vary from duct-preserving to duct-dividing depending on severity.
