A surgery that incises the lateral canthus (outer eye corner) area to widen the horizontal eye width outward.

Overview

Lateral canthoplasty incises the outer corner of the eye (lateral canthus) to extend the horizontal eye width outward. It corrects small eyes or upturned eye corners (fierce-looking impression). Both conjunctival and skin approaches exist, and adhesion-related regression is possible, making surgeon technique critical.

Mechanism

The surgeon incises the lateral canthal tendon and surrounding tissue to release the outer eye corner, then repositions and sutures the conjunctiva and skin to maintain the new, wider eye opening. The conjunctival approach minimizes visible scarring, while the skin approach allows more dramatic widening. Careful suturing prevents excessive adhesion and regression back to the original position.

Effects

Wider horizontal eye width, softer eye shape, correction of upturned or fierce-looking eyes, larger overall eye appearance, and improved facial harmony.

Duration

Permanent, though partial regression (adhesion) occurs in some cases; stabilizes at 3-6 months

Recovery

Swelling and bruising for 1-2 weeks. Stitches removed at 5-7 days. The outer corner appears red and raw initially, healing over 2-4 weeks. Avoid rubbing eyes. Final result at 2-3 months. Eye drops recommended to prevent dryness.

Side Effects

Temporary swelling, bruising, and redness at outer corner. Risk of regression due to adhesion (most common), visible scarring, conjunctival exposure, asymmetry, and dry eyes. Some patients require a second procedure if regression occurs.
