Overview

Lower blepharoplasty corrects sagging skin, fine wrinkles, and protruding orbital fat of the lower eyelid. It can be performed through an external skin incision (below the lash line) or a transconjunctival incision (internal approach), and is often combined with lower eyelid fat repositioning. It improves under-eye wrinkles and puffiness for a more youthful, refreshed appearance.

Mechanism

Through external approach: incision below the lash line allows excision of excess skin and muscle, access to orbital fat for removal or repositioning, and skin tightening. Through transconjunctival approach: internal incision accesses fat only without skin excision, ideal for younger patients with fat herniation but no skin laxity. Combination approaches address both skin and fat components for comprehensive lower eyelid rejuvenation.

Recovery

Side Effects

Temporary swelling, bruising, and tightness. Risk of ectropion (lower eyelid pulling down), scleral show, dry eyes, asymmetry, under-correction, and visible scarring (external approach). Conservative approach recommended to protect eyelid position.
