A surgery that lowers the lower eyelid to increase the vertical height of the eye opening.
Overview
Lower eyelid retraction surgery adjusts the lower eyelid retractors through a conjunctival incision (internal approach) to lower the lower eyelid position. This increases the vertical height of the eye for a larger, more defined eye appearance. Often performed simultaneously with lateral canthoplasty (combined procedure). Over-correction can cause scleral show (visible white below the iris), requiring careful surgical planning.
Mechanism
Through a transconjunctival approach, the surgeon accesses and modifies the lower eyelid retractors (capsulopalpebral fascia) to allow the lower eyelid to sit at a lower position. Spacer grafts (conjunctiva, cartilage, or synthetic material) may be used to maintain the lowered position and prevent relapse. The goal is to expose more of the lower iris while maintaining natural eyelid function and contact with the globe.

Recovery

Side Effects
Temporary swelling, bruising, dryness, and tightness. Risk of scleral show (excessive white showing below iris), ectropion (outward turning of eyelid), dry eyes, asymmetry, and regression. Over-correction is the most concerning risk.
