Nasal Tip PlastySurgery

A surgery that reshapes the nasal tip (nasal apex) in terms of shape, height, and angle using autologous cartilage for a refined appearance.

Overview

Nasal tip plasty approaches the nasal tip through a columellar incision (open approach) or endonasal incision (closed approach) to reposition and suture the alar cartilages, with autologous cartilage grafts (ear cartilage, septal cartilage) as needed. It is performed when the nasal tip is round, bulbous, or drooping. It can be done as a standalone procedure or combined with dorsal augmentation for comprehensive rhinoplasty.

Mechanism

The alar (lower lateral) cartilages are exposed, reshaped through suture techniques (interdomal, transdomal, columellar strut), and reinforced with cartilage grafts. Tip-defining grafts and shield grafts create projection and definition. Cephalic trimming of the alar cartilage reduces tip bulbousness. The cartilage framework is reconstructed to achieve the desired tip shape, projection, rotation, and symmetry.

Refined and defined nasal tip, improved tip projection and rotation, correction of bulbous or drooping tip, improved nasal proportion, and more elegant nasal profile.

Duration

Permanent; cartilage grafts maintain shape long-term with minimal resorption

Recovery

Nasal splint for 5-7 days. Swelling peaks at 2-3 days. Major swelling resolves in 2 weeks. Tip swelling (the last area to settle) gradually improves over 6-12 months. Avoid glasses and nose-blowing for 4-6 weeks.

Side Effects

Temporary swelling, bruising, nasal congestion, and numbness. Risk of asymmetry, graft displacement, infection, cartilage warping, and unsatisfactory shape. Tip skin thickness affects result visibility (thick skin hides refinement).
