Overview

The non-incision method (burial/suture technique) inserts sutures through tiny punctures without cutting the skin, forming adhesions between the upper eyelid skin and the tarsal plate (or levator aponeurosis). It is suitable for patients with thin skin and fat and minimal eyelid drooping. Compared to the incision method, it offers less swelling, faster recovery, and more natural results, but carries a higher risk of fold loosening over time.

Mechanism

Fine sutures are passed through small puncture holes in the eyelid skin, weaving between the skin, orbicularis muscle, and tarsal plate to create controlled adhesion points. When the eyes open, the suture-created connection pulls the skin inward along the designed fold line, mimicking a natural double eyelid crease. The technique relies on scar tissue formation around the sutures to maintain the fold long-term.

Creation of a natural-looking double eyelid fold, larger and brighter eye appearance, improved eye symmetry, and more defined eyelid crease.

Duration

Permanent in ideal cases, but fold loosening occurs in approximately 10-20% of patients over 5-10 years; revision possible if needed

Recovery

Swelling peaks at 2-3 days and significantly subsides within 1 week. Mild residual swelling for 2-4 weeks. Stitches removed at 3-5 days (or absorbable sutures used). Makeup possible after 5-7 days. Natural appearance achieved at 1-3 months.

Side Effects

Temporary swelling, bruising, and mild discomfort. Risk of fold loosening (most common), asymmetry, suture exposure or palpability, infection, and unnatural fold appearance. Revision surgery may be needed if fold loosens.
