A surgery to correct protruding upper and lower jaw by performing anterior segmental osteotomy (ASO) or orthognathic surgery.

Overview

Protruding mouth (bimaxillary protrusion) is a condition where the alveolar bone (the bone supporting the teeth) protrudes forward, making the mouth appear prominent in profile. Anterior segmental osteotomy (extracting premolars and moving the anterior alveolar bone backward) is the representative technique. If the entire skeletal structure is involved, orthognathic surgery may be required instead.

Mechanism

Anterior segmental osteotomy (ASO) extracts premolars, then cuts and repositions the anterior alveolar bone segment backward to reduce protrusion. If the entire jaw is involved, full orthognathic surgery (Le Fort I or BSSO) may be required. The bone segment is fixed with titanium plates in the retracted position.

Effects

Reduced mouth protrusion in profile, improved lip posture and competence, more balanced facial proportions, improved smile aesthetics, and correction of dental protrusion.

Duration

Permanent. Bone healing at 6-8 weeks. Orthodontic finishing may take 6-12 months. Final soft tissue change at 3-6 months.

Recovery

Swelling for 2-3 weeks. Liquid to soft diet for 2-4 weeks. Numbness around chin and lower lip for weeks to months. Return to work at 1-2 weeks. Full recovery at 2-3 months.

Side Effects

Numbness of chin and lower lip (temporary or rarely permanent), infection, tooth root damage, bone non-union, relapse, and gingival recession. Extraction gaps require orthodontic closure.
