A surgery that corrects ptosis (droopy eyelids) caused by weak levator muscle function, making the eyes appear more open and alert.

Overview

Ptosis correction addresses blepharoptosis, where the levator muscle or its aponeurosis is weak and the eyelid does not open fully. The most common technique is levator aponeurosis advancement (incisional or non-incisional), often performed simultaneously with double eyelid surgery. It improves eye size, alertness, and corrects the sleepy-eyed appearance.

Mechanism

The levator aponeurosis (the tendon connecting the levator muscle to the tarsal plate) is surgically shortened or reattached at a more advanced position on the tarsal plate. This increases the mechanical advantage of the levator muscle, allowing the eyelid to open wider with the same muscle effort. In non-incisional approaches, sutures are used to plicate the levator through small punctures.

Effects

Wider eye opening, correction of sleepy or tired appearance, improved visual field, brighter and more alert eyes, and enhanced eye symmetry.

Duration

Permanent; mild regression possible in some cases over years; touch-up rarely needed

Recovery

Similar to double eyelid surgery: swelling peaks at 3-5 days, significant improvement at 1-2 weeks, stitches removed at 5-7 days. Eyes may feel dry initially. Final result at 3-6 months. Lubricating eye drops recommended during recovery.

Side Effects

Temporary swelling, bruising, and dry eyes. Risk of over-correction (lagophthalmos - incomplete eye closure), under-correction, asymmetry, and dry eye syndrome. Over-correction may require revision. Careful preoperative assessment is critical.
