Overview

Mechanism

A septal extension graft is created using harvested cartilage (septal, ear, or rib) and attached to the existing septum to extend its length caudally. The alar cartilages are then repositioned and sutured to this extended framework, rotating the tip downward and forward. This lengthens the nose, reduces nostril visibility from the front, and improves the nasolabial angle. Rib cartilage is preferred for significant extension due to its strength and volume.
