Overview

Square jaw reduction osteotomizes and resects the protruding mandibular angle bone, combined with outer cortex osteotomy and masseter muscle reduction, to soften the angular jawline. Accessed through intraoral incisions, there are no external scars. The procedure requires general anesthesia and careful surgical planning.

Mechanism

Through an intraoral approach, the surgeon exposes the mandibular angle and performs osteotomy using reciprocating saws or burrs. The protruding angle bone is resected, and the outer cortex of the mandibular body may be additionally shaved (cortical osteotomy) for further slimming. If the masseter muscle is hypertrophic, partial resection or Botox may be added. The bone is smoothly contoured to create a natural transition from the chin to the angle area.

Recovery

Side Effects

Significant swelling and bruising. Temporary or prolonged jaw numbness (inferior alveolar nerve). Risk of asymmetry, nerve injury, excessive or insufficient bone removal, sagging jowls, tooth damage, infection, and hematoma. Careful preoperative 3D CT planning is essential.
