A surgical technique that performs a T-shaped osteotomy of the chin to reduce width while simultaneously adjusting length and projection.

Overview

T-osteotomy cuts the chin bone in a T-shaped pattern, removes a central bone segment, and brings the two sides together for fixation. It is suitable for patients with a wide and long chin, and is commonly combined with square jaw reduction to create a V-line contour.

Mechanism

The surgeon makes a horizontal osteotomy below the mental foramen level, then a vertical midline cut creating a T-shape. The central wedge of bone is removed, and the two lateral segments are brought together (medialized), effectively narrowing the chin. The inferior segment can also be advanced, set back, or shortened as needed before fixation with titanium plates and screws. This allows simultaneous control of chin width, length, and projection.
