A device that delivers ultrasound (HIFU) energy deep into the skin (SMAS layer) to achieve lifting and elasticity improvement.

Overview

Ultherapy is a HIFU (High-Intensity Focused Ultrasound) based lifting device developed by Merz (USA). It received the first FDA approval for a non-invasive lifting device in 2009, with FDA indications for brow lifting, jawline lifting, neck wrinkle improvement, and décolletage wrinkle improvement. It delivers thermal energy to the deep layers of the skin (dermis and SMAS fascia) without damaging the skin surface, inducing collagen regeneration and tissue contraction.

Mechanism

Ultherapy focuses ultrasonic energy to create thermal coagulation points at specific depths within the skin (1.5mm, 3.0mm, 4.5mm) at 65–70°C. This thermal energy contracts collagen fibers in the dermal and SMAS (Superficial Musculoaponeurotic System) layers, followed by new collagen production during the natural wound healing process. The SMAS layer is the same tissue that is tightened in a surgical facelift—the ability to deliver energy to this layer non-invasively is Ultherapy's key differentiator. Real-time ultrasound imaging allows the practitioner to visualize skin layers during treatment for high precision.

Effects

Immediate tightening is noticeable after treatment, with progressive lifting effects appearing over 2–3 months as collagen regenerates. Peak results occur at 3–6 months post-treatment, with effects lasting 1–2 years depending on the individual. Key benefits include improved forehead and brow drooping, cheek and jawline lifting, neck wrinkle reduction, and overall skin elasticity enhancement.

Duration

60–90 minutes for a full face. Partial treatments (jawline, eye area) take approximately 30–45 minutes.

Recovery

As a non-invasive procedure, there is virtually no downtime. Slight redness may occur immediately after treatment but disappears within a few hours, and makeup and daily activities can resume immediately. Some patients may experience soreness or tenderness in the treated area for 2–4 weeks.

Side Effects

A sharp or hot sensation is felt as ultrasound energy is delivered during treatment. Pain levels vary by individual and are stronger near bony areas (forehead, jawline). Temporary swelling, redness, and bruising may occur. Rarely, temporary numbness or nerve stimulation symptoms may appear but typically resolve within a few weeks. Treatment should be avoided on areas with metal implants or active acne/wounds.

Price Range

1–3 million KRW for a full face. Prices vary by the number of lines (shots), with 300–800 lines being typical. Partial treatments (jawline, eye area) cost approximately 300,000–800,000 KRW.

Target Audience

Suitable for those experiencing early skin sagging who want non-surgical lifting, those with a loosening jawline and drooping cheeks, those seeking lifting and tightening without surgery, and those who have had thread lifting or Thermage but need additional lifting. For severe sagging, a surgical facelift may be more effective.
