A surgery that removes excess drooping upper eyelid skin and muscle to restore the visual field and achieve a younger appearance.

Overview

Upper blepharoplasty removes excess skin and orbicularis muscle from the upper eyelid that has drooped due to aging, causing visual obstruction or a heavy-looking appearance. The incision follows the double eyelid crease line so the scar is naturally concealed. It serves both functional (visual field restoration) and cosmetic (rejuvenation) purposes.

Mechanism

The surgeon marks the amount of excess skin to be excised (pinch test), incises along the existing or desired crease line, removes the redundant skin and a strip of orbicularis muscle, addresses any protruding orbital fat if needed, and closes the incision with fine sutures. The resulting scar falls within the eyelid crease and becomes virtually invisible once healed.

Removal of excess upper eyelid skin, improved visual field, lighter and more open eyes, rejuvenated and alert appearance, and reduction of eyelid heaviness.

Permanent; natural aging will eventually cause recurrence of skin laxity over 10-15+ years

Recovery

Swelling and bruising for 1-2 weeks. Stitches removed at 5-7 days. Makeup possible after stitch removal. Mild residual swelling for 1-2 months. Scar fades to a fine line concealed in the crease over 3-6 months.

Side Effects

Temporary swelling, bruising, and tightness. Risk of asymmetry, over-excision (difficulty closing eyes), visible scarring, dry eyes, and lagophthalmos. Conservative excision is preferred to avoid over-correction.
