Overview

V-line surgery combines mandibular angle resection (square jaw reduction) with chin T-osteotomy (or horizontal osteotomy) and advancement/reduction to reshape the entire lower face into a V-line. It produces dramatic changes in lower facial contour and, as a complex combined surgery, requires extended operating time and thorough preoperative planning with 3D CT.

Mechanism

The surgeon performs mandibular angle osteotomy and resection bilaterally to reduce jaw width, then performs a T-shaped or horizontal osteotomy of the chin (mentum) to narrow and reshape the chin point. The chin segment may be advanced, set back, shortened, or narrowed as needed, and is fixed with titanium plates. The combined reshaping creates a continuous V-line contour from ear to chin when viewed from the front.

All risks of both square jaw and chin surgery combined: significant swelling, prolonged numbness, asymmetry, nerve injury (mental nerve, inferior alveolar nerve), tooth damage, sagging, infection, non-union, and blood loss. Longest recovery among facial contouring procedures.
