Overview

Milia are 1-2mm white or yellowish-white cysts formed when keratin becomes trapped in a capsule beneath the epidermis. Common around the eyes, cheeks, and forehead, they rarely resolve spontaneously and usually persist. Treatment involves extraction using a sterile needle (18-gauge), CO2 laser vaporization, or electrocautery, all of which are simple outpatient procedures.

Mechanism

A sterile needle or lancet is used to create a tiny opening in the overlying epidermis, and the encapsulated keratin contents are expressed. Alternatively, CO2 laser precisely vaporizes the thin epidermal roof to expose and remove the cyst contents. Electrocautery uses thermal energy to open and destroy the cyst wall. All methods aim to completely remove the keratin capsule to prevent recurrence.
