A 10,600nm carbon dioxide laser that is the oldest and most widely used general-purpose device in dermatology.

Overview

CO2 laser uses the 10,600nm wavelength to vaporize (ablate) tissue. It is fundamentally used for mole removal, wart removal, seborrheic keratosis, syringoma, and milia removal. In fractional mode, it treats scars, pores, and performs skin resurfacing. As the most versatile laser in dermatology, virtually every clinic has one.

Mechanism

The 10,600nm wavelength is strongly absorbed by water in tissue, causing rapid heating and vaporization of the target area. In continuous or pulsed mode, it precisely ablates tissue layer by layer for lesion removal. In fractional mode, it creates microscopic ablative columns surrounded by intact tissue, triggering wound healing, collagen remodeling, and skin renewal. The depth and density of ablation are controlled by adjusting power, pulse duration, and scanning pattern.

Precise tissue ablation for lesion removal (moles, warts, skin tags), skin resurfacing, acne scar improvement, pore reduction, wrinkle reduction, and overall skin rejuvenation in fractional mode.

Duration

Lesion removal: often single session; fractional resurfacing: 1-3 sessions at 4-8 week intervals; results from fractional treatment continue improving for 3-6 months

Recovery

Lesion removal: wound healing for 7-14 days with ointment care. Fractional: redness, oozing, and crusting for 5-10 days; full redness resolves over 2-4 weeks. Sun avoidance critical. Wound care and moisturizing essential during healing.

Side Effects

Temporary redness, crusting, oozing, and discomfort. Risk of post-inflammatory hyperpigmentation (especially in Asian skin), scarring if wound care is inadequate, infection, and delayed healing. Fractional mode has less risk than full ablative but still requires careful aftercare.
