Ruby Laser

Overview

The ruby laser uses the 694nm wavelength, which has very high melanin absorption, making it excellent for removing epidermal pigmented lesions such as spots, freckles, and seborrheic keratosis. However, the high melanin affinity also means increased risk of post-inflammatory hyperpigmentation in darker skin tones, requiring careful patient selection and parameter adjustment.

Mechanism

The 694nm wavelength is highly absorbed by melanin with relatively low absorption by hemoglobin, providing excellent selectivity for pigmented lesions. In Q-switched mode, nanosecond pulses fragment melanin with high efficiency at the epidermal level. The strong melanin absorption means effective treatment with lower energy, but also requires caution with darker skin types due to increased risk of epidermal melanin damage.

Highly effective removal of freckles, sunspots, and seborrheic keratosis; treatment of superficial pigmented nevi; excellent cosmetic outcomes for fair-skinned patients; and precise targeting of melanin-rich lesions.

Duration

1-3 sessions for most lesions; 4-6 week intervals; some spots clear in a single session

Recovery

Treated spots darken and form crusts that fall off in 7-10 days. Mild swelling for 1-2 days. Wound care with ointment and band-aid protection recommended. Strict sun avoidance for 4+ weeks to prevent post-inflammatory hyperpigmentation.

Side Effects

Temporary darkening and crusting. Higher risk of post-inflammatory hyperpigmentation in darker skin types (Fitzpatrick IV-VI). Hypopigmentation possible. Not recommended for melasma. Scarring risk if aftercare is inadequate.
