A skin tightening device that delivers radiofrequency (RF) energy deep into the skin to induce collagen contraction and regeneration.

Overview

Thermage is an RF (radiofrequency) based skin tightening device developed by Solta Medical (now Bausch Health, USA). It received FDA approval in 2002, and the most widely used model is Thermage FLX (4th generation). Using monopolar RF energy, it delivers heat uniformly throughout the dermal layer, inducing collagen contraction and neogenesis to improve skin elasticity. It can be applied to the face as well as the eye area, body (abdomen, thighs, upper arms), and buttocks.

Mechanism

Thermage's monopolar RF energy flows from the tip on the skin surface to a grounding pad on the back, generating heat within the skin tissue. This thermal energy (typically 60–70°C) causes immediate contraction of collagen fibers in the dermal layer, followed by new collagen and elastin production through the wound healing response over several months. Thermage FLX (4th generation) features AccuREP technology that measures skin impedance with each shot and automatically adjusts energy delivery for improved uniformity and safety. Unlike Ultherapy, Thermage delivers heat broadly and uniformly across the entire dermis rather than targeting the SMAS layer.

Effects

Immediate tightening from collagen contraction is felt after treatment, with gradual elasticity improvement from collagen regeneration over 3–6 months. Key effects include overall skin elasticity and texture improvement, pore reduction, and fine wrinkle softening. Effects typically last 1–2 years, with annual maintenance treatments recommended. While Ultherapy excels at "lifting," Thermage is stronger for "tightening and elasticity"—the two are often combined.

Duration

30–60 minutes for a full face. Thermage FLX reduced treatment time by approximately 25% compared to previous generations. Eye area treatment (Thermage Eye) takes about 15–20 minutes.

Recovery

As a non-invasive procedure, there is virtually no downtime. Temporary redness may appear immediately after treatment but disappears within a few hours, and makeup and daily activities can resume right away. Swelling and bruising are rare.

Side Effects

Warmth and mild discomfort are felt during treatment. Thermage FLX features vibration and cooling systems that significantly reduce pain compared to earlier generations. Excessive energy settings may cause subcutaneous fat atrophy (volume loss), so energy must be carefully adjusted for patients with thin cheeks. The treatment is contraindicated for patients with metal implants (excluding dental implants) or pacemakers.

Price Range

Approximately 800,000–2,000,000 KRW for a full face (Thermage FLX, 900 shots). Prices vary by shot count (300/600/900). Thermage Eye costs approximately 300,000–600,000 KRW. Body Thermage ranges from 1–3 million KRW depending on the area.
